Deactivating an Affiliation Agreement (Providers) 1. From the left-hand menu, use the drop-down to select the Rotation Period and Provider. The selected rotation period will be applied across all modules and screens.2. From the left-hand menu, select Provider Details, then click Profile. 3. On the Manage Profile screen, click the + icon next to Agreements.4. Find the Affiliation Agreement you want to deactivate and click Deactivate.5.
