Adding a Provider Affiliation Agreement There are two ways to add an affiliation agreement. Option 1:1. In the left navigation menu, select Home.2. In the Quick Actions menu on the right, click Setup Affiliation Agreements.3. In the popup, select the Provider and School from the dropdown menus, fill in the necessary details for the affiliated provider site, and you'll have the option to upload a document to the record.Uploading a document is optional. 4. Click Save.5. After verification, the affiliation will be listed in the Affiliation Agreements section below Quick Actions. Option 2: 1. From the left navigation menu, select Profile. 2. Make sure your provider site is selected in the center of the screen. If it isn’t, click the + icon and use the search filters to find your provider. 3. Click Add. 4. Enter or update the required details:SchoolPlacement TypeExpiration DateUpload any relevant agreements5. Click Save to complete the process.
