Provider Adding a School Affiliation Agreement 1. From the left-hand menu, use the drop-down to select the Rotation Period and Provider. The selected rotation period will be applied across all modules and screens.In the left-hand navigation bar, click Provider Details, then select Profile.3. On the Manage Profile screen, choose the School you want to add an affiliation agreement for, then click Edit.4. In the Create or Update Affiliation Agreement pop-up window:Enter the required information for the new agreement.Click Upload Agreement to attach a copy of the agreement.Click Save. After saving, return to the Manage Affiliation Agreements screen. Scroll to the bottom of the page and click Save again.A confirmation pop-up will appear to confirm the agreement has been saved.
